Video Release Form

I hereby give permission for video images of myself and my family, taken on library property by library staff or library-hired videographers, to be used solely for the purposes of promoting the library, and waive any rights of compensation or ownership thereto. I give permission for these video images to be posted online, on the library's YouTube account, on local public access channels and on the library's websites, for library promotional purposes only.
